Five tablets is a deliberate dose, as together they provide 5 g of creatine monohydrate per day – the upper limit of the maintenance dose recommended for long-term use by the International Society of Sports Nutrition (Kreider et al., 2017). A smaller number of tablets would mean a lower daily intake, which isn't necessarily wrong (even 3 g/day is effective), but it would extend the time needed to fully saturate the muscles' creatine stores. If taking five tablets at once isn't convenient for you, you can split them throughout the day, since the total daily amount matters more than a single dose.
Elevated serum creatinine is a common and expected physiological occurrence with creatine use, since creatinine is a natural metabolite of creatine – on its own it does not indicate kidney damage (Antonio et al., 2023).
